Kalitta N703CK made her very last flight this Thursday, April 11 2024. The Boeing 747-400F arrived at Detroit Willow Run Airport (YIP) airport from Toronto to be clearing customs before its final flight.

About 6 hours later, the 33 year-old lady made its final flight to be retired. A 1 hour and 45 minutes flight to Oscoda-Wurtsmith Airport (OSC).

Trash bin incident

On Feb 3rd 2020 this cargo plane hah to reject takeoff due to trash bin on runway. The plane was taking off to Honolulu at about 3 a.m. from runway 25R when it ran into the trash bin, which had been blown into the plane’s path by strong winds.

The bin hit the nose gear of the plane and blew out two of its tires. Seven people were on board the plane, but none were injured. The Los Angeles Fire Department said there was no fuel leak or fire.

The captain stated that he was the pilot flying and, during the takeoff roll on runway 25R, between 80 and 110 knots (V1 was 131 knots), he noticed a large object “moving right to left at a good speed”. A fraction of a second later it passed under the nose and impacted the airplane. He said that the impact was loud, and the airplane “settled”, followed by severe vibration in the airplane. He immediately rejected the takeoff and stopped on the runway.

Lufthansa flight #LH402 departed terminal 1 at Frankfurt about one hour late Thursday, April 11 2024.

The flight was operated by a 9 year-old Boeing 747-8 (registration D-ABYR).

For a reason still unclear, while on approach to Newark Airport at 15,000 ft the crew decided to divert to Boston.

The aircraft landed on runway 4R after 9 hour-flight at 6:00pm EDT.

But 18 hours later, it looks like passengers are still stranded in Boston. The Boeing aircraft is still on the ground at Logan International Airport.

As a result, the engine emitted debris through the exhaust, which landed in the village of Meerssen, causing injuries to two people and property damage to houses and cars. The flight crew shut down the engine and landed safely at Liège Airport in Belgium.

The Dutch Safety Board classified the incident as serious because of the chance of an accident. This is because the falling debris ended up in a village and caused the injury, reported onderzoeksraad.nl.

That injury could have been much more serious. By the Dutch Safety Board Kingdom Act and European Regulation No. 996/20102, the Dutch Safety Board investigated this serious incident.

Report by the Dutch government

The Dutch Safety Board investigated the risk posed to individuals on the ground following concerns raised by residents through letters and emails.

The concerns centered on the safety of the residents after injuries and property damage occurred as a result of the incident above. Residents rely on the government to manage risks associated with the Maastricht Aachen Airport for their safety.

The airport is perceived as unsafe due to its location between residential areas and its use for commercial air transport services, including flights with heavy cargo aircraft. The incident has further exacerbated the residents’ sense of insecurity.

As per ICAO Annex 13 guidelines, the survey included the following organizations: the Dutch Safety Board, which represented the country where the incident occurred.

The National Transportation Safety Board (NTSB) represented the country of design and manufacture of the Boeing 747 and the Pratt & Whitney PW4056 engine type.

In addition, the NTSB has appointed designated representatives and advisors to Boeing Commercial Airplanes, Pratt & Whitney, and the United States Federal Aviation Administration.

The British civil aviation investigation body, the Air Accidents Investigation Branch (AAIB), represented the airline’s country and the country of registration and appointed an authorized representative and advisor from the airline Longtail Aviation.

The report is in Dutch, and it states the various reasons why it occurred and what led to this failure. They have included data from the engine manufacturers and also added the readings.

Lufthansa (LH) Boeing 747-8 took off from Frankfurt at 10:50 CEST and crossed Ireland. However, the pilot decided to return to Frankfurt amid mechanical problems associated with the hydraulic system.

After dumping fuel over the Celtic sea to avoid heavy landing, flight LH462 returned to Frankfurt Airport. The Radarbox spotted the aircraft track and shared them on Twitter.

Lufthansa Boeing 747-8 Emergency landing

Avherald reported the incident and stated, During its flight from Frankfurt/Main, Germany to Miami, FL, USA, a Lufthansa Boeing 747-8 aircraft with registration D-ABYG encountered a hydraulic problem while en route to Oceanic Airspace over the Atlantic Ocean, about 220 nautical miles northwest of Shannon, Ireland.

As a result, the crew decided to return to Frankfurt. After receiving a revised flight plan from dispatch, the plane climbed to FL350 but had to dump fuel at altitude as it was too heavy to land in Frankfurt.

The aircraft descended to FL310 for the fuel dump and was then vectored to fly southwards before the coast of Ireland.

After dumping the fuel, the plane returned to Frankfurt and landed with open gear doors on runway 07C. The plane landed approximately 2 hours and 40 minutes after deciding to return to Frankfurt.

Boeing 747-8 Problems

Earlier in 2014, Federal Aviation Administration (FAA) issued a new airworthiness directive for specific Boeing 747-8 and 747-8F planes powered by certain General Electric Co engines.

The directive mandates the replacement of faulty software with an improved version. This was the fourth such directive issued for the 747-8, and it applied to seven aircraft in the US.

Similar to the latest incident, A Polar Air Cargo Boeing 747-8 freighter, registration N857GT performed flight PO-213 from Cincinnati, KY (USA) to Tokyo Narita (Japan). When it was nearing the top of descent towards Tokyo about 40 minutes prior to the estimated landing when the crew reported a hydraulic failure.

The aircraft continued for a safe landing on runway 16R and stopped on the runway with open gear doors.

Another incident involved QR, Qatar Airways Boeing 747-8F suffered the hydraulic system 1 failure when the aircraft was descending towards Doha. The hydraulic system failure had depleted all its hydraulic fluid.

The three-year-old Boeing 747-8F, with registration A7-BGB, was inspected by the engineers. The inspection determined that engine number 1 needs to be replaced.
